    What Is Bevacizumab and How Does It Work Against Recurrent Glioblastoma?

    Bevacizumab (Avastin) is a drug approved for recurrent glioblastoma that blocks VEGF, a protein glioblastoma uses to grow new blood vessels. By cutting off that supply, it can shrink the visible tumor on MRI and reduce brain swelling. It does not kill cancer cells directly. That distinction matters when tracking how well the drug is working and when to consider switching.

    The US National Cancer Institute lists bevacizumab as an approved therapy for recurrent glioblastoma. The FDA gave it accelerated approval in May 2009, based on response rates from two phase II clinical trials. Full approval followed in 2017. The drug is marketed under the brand name Avastin, with biosimilar versions now available in many countries.

    Who Is Typically Offered Bevacizumab?

    Bevacizumab is rarely given as part of initial treatment. Most neuro-oncologists use it at recurrence - after the tumor comes back following surgery, radiation, and temozolomide chemotherapy. At that point it is one of a limited number of options that may meaningfully control disease.

    Before starting, your oncologist will check for conditions that raise the risk of serious side effects: uncontrolled high blood pressure, a recent surgical wound, a history of blood clots, or significant kidney problems. Patients who had brain surgery within the past few weeks face a higher risk of bleeding or poor wound healing on bevacizumab, so timing is part of the decision.

    Bevacizumab can be particularly useful when brain swelling (called cerebral edema) is causing symptoms such as headaches, limb weakness, or cognitive changes. In many cases it allows the dose of dexamethasone (the steroid commonly used to control swelling) to be reduced.     What Does the Evidence Actually Show?

    The pivotal data come from two phase II trials that formed the basis for FDA approval. In those studies, bevacizumab alone produced an objective response rate - meaning the tumor visibly shrank or disappeared on MRI - of roughly 28 to 43 percent. Studies show that the six-month progression-free survival rate for bevacizumab alone was approximately 42.6 percent, compared to around 15 percent for recurrent GBM treated with older approaches. Median overall survival was approximately 9.2 months.

    These numbers describe averages across a broad population. Some patients respond for considerably longer; others progress within the first two cycles. Performance status, tumor volume at the time of starting bevacizumab, and the molecular profile of the tumor all influence individual outcomes.

    Despite encouraging response rates, large randomized trials have not shown that bevacizumab significantly extends overall survival in recurrent GBM. What it does offer many patients is meaningful symptom control - reduced brain swelling, shrinkage of visible tumor on standard MRI, and sometimes improved or stabilized neurological function - for a window of time that can matter in daily life.

    What Is Pseudoresponse and Why Does It Complicate Tracking?

    Pseudoresponse is one of the most important concepts to understand when a patient is on bevacizumab. It happens when the MRI looks like the drug is working - the contrast-enhancing area (the bright region seen after a contrast dye injection) appears to shrink - but the drug is really just reducing the leakiness of blood vessels, not killing the tumor. The cancer itself may still be growing, now visible only on a different MRI sequence called FLAIR (fluid-attenuated inversion recovery).

    Studies show how bevacizumab can dramatically reduce contrast enhancement on standard MRI while non-enhancing tumor burden on FLAIR imaging actually increases. Standard response criteria - which traditionally rely on measuring the contrast-enhancing area - can therefore be misleading in patients receiving bevacizumab.

    At every MRI appointment, ask your neuro-oncologist whether both the enhancing and non-enhancing (FLAIR) regions of the tumor are being measured and compared to prior scans.     What Are the Main Side Effects?

    Bevacizumab has a distinct side effect profile compared to standard chemotherapy. The most common effect is high blood pressure (hypertension). High blood pressure during bevacizumab may signal that the drug is working - though it still requires treatment and close monitoring at every visit.

    Other effects to know about:

    • Fatigue, which can be significant and may affect sleep and daily activity
    • Proteinuria - protein in the urine, checked with a simple dipstick test before each infusion cycle
    • Impaired wound healing, which is why bevacizumab is typically paused before and after any planned surgical procedure
    • Venous thromboembolism - blood clots in the deep veins of the leg or in the lungs
    • Rare but serious: intracranial hemorrhage (bleeding into the brain)
    • Rare but serious: gastrointestinal perforation

    Bevacizumab is given by intravenous infusion, typically every two to three weeks. Blood pressure, urine protein, and blood counts are checked regularly throughout treatment. Many patients report fatigue and sleep problems on bevacizumab. Tell your care team about these side effects so they can help.

    How Long Does Bevacizumab Typically Keep Working?

    In clinical studies, the median time to tumor progression on bevacizumab alone was approximately 6.1 months. Half of patients progressed before that point; half responded longer. Individual experience varies considerably, and some patients maintain disease control for more than a year.

    Glioblastoma tumors almost always find a way around bevacizumab over time. The cancer can recruit alternative blood vessel pathways that are not blocked by VEGF inhibition, shift to an infiltrative growth pattern that does not depend on new vessels, or activate parallel signaling routes that sustain tumor growth despite VEGF blockade. This acquired resistance is the central limitation of bevacizumab in GBM, and progression after an initial response is expected.

    How Do You Know When to Switch to a Different Therapy?

    Recognizing real progression on bevacizumab is difficult. Because of pseudoresponse, a scan that looks stable or improved may mask genuine tumor growth in the non-enhancing region. The following signals, taken together, suggest the drug may no longer be controlling disease:

    • New or worsening neurological symptoms - weakness, speech problems, increased seizure frequency, or significant cognitive decline - that cannot be explained by medication side effects alone
    • Growth of the FLAIR region on MRI, even if the contrast-enhancing area appears stable or smaller
    • New contrast-enhancing lesions appearing in a different part of the brain
    • A steady decline in physical or cognitive function across two or more consecutive clinic visits
    • Confirmed growth on two consecutive imaging studies using consistent measurement criteria

    There is no universal threshold that automatically triggers a switch. The decision comes from a careful conversation between the patient, caregiver, and neuro-oncologist, weighing imaging findings against how the person is actually functioning day to day.     What Are the Options After Bevacizumab Fails?

    Progression after bevacizumab is a difficult stage in treatment. Data suggest median overall survival after bevacizumab failure is in the range of four to five months in many reported series. But patients do respond to further treatment, and several options exist worth discussing.

    Options that neuro-oncologists may evaluate include:

    • Lomustine (CCNU): An oral alkylating chemotherapy sometimes used at second or later recurrence, either alone or in combination with another agent.
    • Re-irradiation: Stereotactic radiosurgery or hypofractionated radiation delivered to a well-defined focal recurrence.     • Regorafenib: An oral multikinase inhibitor that has shown activity in some recurrent GBM studies, though evidence remains limited and use varies by center and country.
    • Tumor Treating Fields (TTFields): A wearable device that delivers low-intensity alternating electric fields designed to disrupt tumor cell division. TTFields have been combined with bevacizumab in recurrent GBM trials and may be initiated or continued as part of a salvage strategy.
    • Clinical trials: At bevacizumab failure, a clinical trial is often the most important option to pursue seriously. Immunotherapy approaches - including checkpoint inhibitors and personalized tumor vaccines - are actively being studied.     No large randomized trial has yet established a clear standard of care after bevacizumab failure. This is why access to an experienced neuro-oncology center and an active clinical trial network matters most at this point in treatment.

    Should Bevacizumab Be Continued After the Tumor Progresses?

    Some neuro-oncologists discuss adding a second agent to bevacizumab at the point of confirmed progression rather than stopping it completely. The reasoning is that abruptly discontinuing bevacizumab can trigger rebound edema and rapid neurological deterioration in some patients. However, the evidence for continuing bevacizumab past confirmed progression is limited, and no approach has clearly demonstrated better outcomes over simply stopping and switching to a new therapy. This decision requires experienced clinical judgment and, ideally, a multidisciplinary tumor board review of all imaging and prior treatment history.

    When to Talk to Your Doctor

    Contact your care team promptly if you notice new or worsening neurological symptoms, blood pressure readings that are unusually high, calf pain or swelling, sudden severe headache, or any change in how a recent surgical wound is healing. At every MRI appointment, ask specifically whether both the contrast-enhancing and FLAIR (non-enhancing) regions of the tumor are being measured and compared to prior scans - not just the bright area on the post-contrast images. Tracking both is the most reliable way to detect real progression rather than pseudoresponse.
